Assesment of the lumbar paraspinal muscles are frequently overlooked on cross sectional imaging. The mulfidus muscles originate from the spinous processes and insert in the facet joints, iliac crests, and sacrum. Atrophy and signal changes in this muscle group are frequently associated with back pain. We will review several cases of primary and secondary causes of multifidus abnormalities, including atrophy and signal changes. Mulfifidus muscle group abnormalities may explain back pain in the absence of other abnormalities.
