Purpose

PURPOSE
CT guided access to majority of larger or posteriorly located lesions in the sacral area is usually straightforward. Access to those of smaller size or anterior location is more complicated and requires careful plotting of trajectory path. The purpose of this presentation is to discuss the planning of approach and execution of the biopsy, based on location (parasacral, bone, intra-canal), character (blastic, lytic) of the lesion and choice of the target in multifocal disease.

Methods & Materials

1. Anatomy of the sacrum presented, using drawings and cross sectional diagrams, with emphasis on bony landmarks, major vascular channels and neural pathways.
2. Input from diagnostic studies (MR, CT, PET) discussed.
3. Planning algorithm suggested, including assignment of a case into one of five groups, from easiest to most difficult.
4. Technical aspects described, including entry point and target calculations.
